Wright’s affordable 10-Year Level Term Rates are based on gender, current age and health. For specific rates click on the applicable rate table link below based on the amount of coverage you are applying for.

You can download, complete, and return the signed application to the Plan Administrator listed on the form. Be sure to answer all the questions on the application. You may be contacted about additional requirements that may be needed to confirm your proof of insurablity.

If You Download the Application - Please Note: Your future Wright USA premiums can be paid automatically from your account at your financial institution on a Monthly, Quarterly, or Annual basis through Electronic Funds Transfer (EFT).
