10-YEAR LEVEL GROUP TERM LIFE - PLAN DETAILS
Eligibility
  • Full or part time Federal Employees working at least 17.5 hours per week ...or a retired employee, under the age of 65, residing in the U.S. (except Vermont), District of Columbia or Puerto Rico may apply.

  • Individuals meeting the criteria directly above may also apply for coverage on their lawful (not legally separated) spouse under the age of 65 & their unmarried, dependent children 14 days old through 19 years old (25 for full-time students).

  • Benefit amounts available for member and spouse are from $25,000 to $1,000,000 in $25,000 increments. A spouse who is an eligible member must apply for member coverage. Spouse amount cannot exceed member amount.

  • Each child can be covered for $5,000 ($500 for children 14 days to 6 months). One premium of $6.00 per year covers all children. If both parents are insured as members, only one may request child coverage.

  • Total combined coverage for each insured person under the Group Term Life Insurance Plans underwritten by New York Life is $1,000,000.
Right to change benefits, rates or terminate the plan
  • Changes to the group policy are subject to agreement between the group policyholder (CSEBA) and New York Life Insurance Company. CSEBA or New York Life may terminate the plan on any premium due date with advance notice. If the plan is terminated by CSEBA or New York Life, at least 30 days advance notice will be provided to the individual insured member(s). Rates after the first 10 years may be changed by New York Life on any premium date and on any date which benefits are changed and only on a class-wide basis. For example, a Class is a group of insureds with the same state, age and gender.
Exclusions
  • Suicide - Suicide is not covered if it takes place during the two-year period after a person’s coverage becomes effective. (Premiums paid for the person’s coverage will be returned.) Missouri residents: Benefits will not be paid for suicide within the first two years of coverage if New York Life can show that suicide was intended at the time of application.

Additional Features

  • A Living Benefit or "Accelerated Death Benefit" (ADB) * - Designed to help terminally ill insureds during a difficult and often financially challenging time & provides the option to have a portion (up to 50%) of your life insurance benefit paid to you while you are still alive. This money can be used to help cover prescription drug costs, medical bills, outstanding debts, experimental treatments, the cost of modifications to your home, or a family vacation. The choice is yours!

    To qualify for the ADB, an individual must be diagnosed as having a life expectancy of 12 months** or less. Complete details are in the Certificate of Insurance.

    Please note that receipt of Accelerated Death Benefits may affect eligibility for public assistance programs and may be taxable. Prior to applying for Accelerated Death Benefits, insureds should consult with the appropriate social services agency, and assistance should be sought from a qualified tax advisor.

    * This benefit is not available to residents of Massachusetts.
    ** 24 months for residents of Illinois

  • Group Conversion Privilege - You may convert your coverage to an individual plan offered by New York Life, regardless of physical condition or medical history, under the conditions stated in your Certificate of Insurance. The right of conversion is also available to your covered dependents.

  • Beneficiary - You can choose anyone you wish as your beneficiary. That person will receive all the proceeds of your coverage in the event of your death. It could be a family member or a close friend. You are automatically the beneficiary for your dependent’s coverage. You can change your beneficiary at any time, by written request. If you wish, you can name another beneficiary for your spouse’s coverage.

  • Survivor Benefit - If an insured member dies, life insurance then in force on his/her dependents may be continued as described in the Certificate of Insurance.

  • Incontestability - The validity of any amount of insurance which has been in force for two years during the insured’s life will not be contested except for eligibility and non-payment of premium contributions.

  • Certificate of Insurance - The information provided here is only a brief description of the principle provisions and features of the plan. The complete terms and conditions are set forth in the certificate of insurance, under group policy G-29155-0/FACE, which will be mailed to you upon approval. This group policy is issued by New York Life Insurance Company to the Civil Service Employee Benefit Association on Policy Form GMR.
