Rates described in this section apply only to eligible Federal Employees. See Plan Details for a complete description of Eligiblity.

Basic Term premiums are based on an affordable flat charge per $1,000 of coverage for eligible applicants and their spouses. Premium details for calculation purposes are as follows:

Current Premium Rates

Employee/Spouse*:	From $25,000 to $300,000 $2.08 per $1,000 of coverage
Child(ren):	$10,000 $18.20 (one rate covers all dependent children)

Example Annual Rates:

Eligible Applicant coverage only - $150,000 - Annual Rate = $312 (i.e., 150 x 2.08)

Eligible Application + Children coverage - $150,000 (Applicant) + $10,000 (on each Child) - Annual Rate - $330.20 (i.e., 150 x 2.08 + 18.20)

Eligible Applicant coverage only - $300,000 - Annual Rate = $624 (i.e., 300 x 2.08)

Eligible Application + Child(ren) coverage - $300,000 (Applicant) + $10,000 (on each Child) - Annual Rate - $642.20 (i.e., 300 x 2.08 + 18.20)

*Rate is for Eligible Applicant and Spouse. Rates for Retired insureds/spouses over age 50 are higher. Contact the plan administrator for details. Rates shown are guaranteed until March 1, 2014.

You can download, complete, and return the signed application to the plan administrator listed on the form. Be sure to answer all the questions on the application.
