You must be a full or part-time Federal Employee (working 17.5 or more hours per week) and residing in the United States (except Vermont) to apply for Basic Group Term Life Insurance.

Please download, complete and return the application to the plan administrator at the address listed on the form. Be sure to answer all questions on the application and initial any changes you make.

You may qualify for this coverage on a Guaranteed Acceptance Basis
If you are a New Hire (i.e., within 45 days of your Hire Date) or have had a Grade Level Change (I.e., within 60 days of your Grade Level Change), you can Download the Guaranted Acceptance Enrollment Form, complete it and return it to the plan administrator at the address listed on the form. Be sure to answer all the questions and initial any changes you make.

New Hires within the first 45 days of your date of hire you can enroll for coverage up to your current Salary (rounded up to the next higher $1,000)

EXAMPLE - If your starting salary is $55,499 - Maximum Coverage Amount available without medical underwriting is $56,000

Grade Level Change If you are already insured under the plan and received a Grade Level Change within the past 60 days - you can increase coverage to a maximum of your current salary amount (but limited to a $50,000 increase on a Guaranteed Acceptance basis.

EXAMPLE - Salary went from $68K to $150K. Before this increase, your Basic Life amount of coverage is $68,000. Since the salary increase is $82K - the maximum you can request on a guranteed acceptance basis is $50K for a total coverage amount of $118,000.
NOTE - If at time of grade level change your coverage amount was $50,000, then the maximum coverage amount you can request is $50,000 for a total coverage amount of $100,000.
