We offer you two convenient ways to apply for our CIGNA Dental Care® (DHMO) Economy Plan: 1.) Online; or 2.) Downloadable Application.

You must pick a network dentist when you apply. Find a dentist near your home/office by using the CIGNA Provider Directory online. Select “Dentist” under “What Type of Provider are you looking for ?”, provide at least the additional required information, and click on the “Next” button. Select “CIGNA Dental Care (HMO)”, then “DHMO Network” and dentist type in the drop-down boxes. You can also call the automated Dental Office Locator at 1-800-244-6224 any time. Whether you apply online or via downloaded application, make sure to indicate your choice of dentist when you apply.
